All package forms must be completed completely and legibly. Submit the enrollment packet, a copy of your child's birth certificate, immunization records, a copy of the parent's photo ID, custody documents (if applicable), and a copy of one of the supporting documents listed in the registration packet for the Arizona residency form via:
